66e6293a92 [spamblock] Remove dead code (Akismet spam toggle) 
This code was used to tell Akismet that a comment
is spam or ham. It can't be triggered as there
are no UI buttons, and it shouldn't be used as
it uses GET params. I don't think it will be
missed.

The relevant issue has been open for about two
and a half years without a new implementation,
so let's drop it. We can always re-implement it
later ...

Closes #405.

2ecd2bdd13 Return output 
When $use_gd is false, returning $output so that the captcha images that have been generated are actually embedded into the final rendering of the page.

Fixes #536.

7410465496 Improve accessibility of iconfont icons 
Iconfont icons are of no value to screenreader users; in our case,
they get alternative text. By adding 'aria-hidden="true"' to the
<span> holding the iconfont icon, we avoid the screenreader trying
to announce the iconfont icon.
